Responsibilities

The Director provides programmatic leadership and direction for United hospital and emergency department Care management (including discharge planning) activities in order to meet strategic goals of Allina Health, regulatory, clinical, and contractual objectives for internal and external providers; state and federal agencies; and managed care payers. The Director assists in developing and implementing strategies to integrate clinical care across the continuum for United Hospital patients, to meet hospital and emergency department care management quality, experience and financial goals, and is responsible for operational requirements of managed care and other payer contracts. This position represents hospital care management as part of the care management and integration leadership team.




Education
  • Bachelor's degree in nursing, social work or health care related field.
  • Master's degree required or actively completing graduate studies in a health related field.

Qualifications

Experience:

  • Minimum 10 years of related healthcare experience.
  • Demonstrated proficiency in managing complex, matrixed operations.
  • 3 years care management experience preferably in an integrated healthcare delivery system.
  • Demonstrated track record in leading and delegating a team with accountability and successful achievement of superior patient care, operations and financial results.
  • Demonstrated experience and desire to effectively work with physicians.
  • Experience in strategic planning, program development, finance, and marketing.
  • Ability to lead
  • Energized, enthusiastic, passionate, and engaged leader with strong intellect and creativity.
  • Experience working with key strategic partners and independent physician groups.

Licensure/Certifications:

  • Medical Terminology
  • Other: Certified Case Manager a plus
  • License Required: Current RN, Social Work or other clinical licensure.

RN hires with less than 1 year of RN experience must be a graduate of an accredited school of nursing (American Association of Colleges of Nursing or The National League for Nursing) which is inclusive of a clinical rotation in an acute care facility.
