Ecumen at Home, the most innovative leader of senior housing and services, is seeking an experienced RN, Home Care Case Manager in the Twin Cities area. As a RN, Home Care Case Manager, you will perform a variety of professional nursing and home health aide duties in the care of clients which comply with state and federal rules and regulations, and follows established policies and procedures of Ecumen at Home. The RN Home Care Case Manager will coordinate client care, conduct nursing visits, complete new admissions, hospital transfers and discharges. This position is responsible for ensuring accuracy and promptness with proper charting practices and assist in the evaluation of job performance of Home Health Aides.

Job Requirements

Education, training, experience:
Registered Nurse, will have and maintain a current, unencumbered Registered Nurse licensure in the State of Minnesota and be familiar with general nursing and geriatric nursing practices. The RN will demonstrate leadership, good judgment, dependability and be knowledgeable and accurate in the administration of medications and treatments. Must understand and communicate with specialized medical terminology and explain procedures to clients. Previous home health experience preferred.
